The “Tangled Roots” (Varicocele & The Broken Radiator)

13 March 2026 3 min readBy Santaan Fertility Center and Research Institute

In the farming and weaving communities of Bharat, long hours of standing are a way of life. But for men, this can lead to “tangled veins” that quietly trap heat and damage the “seeds.” This post explores Varicocele and how XAI helps cool the internal fire to restore male fertility.

The Story: Meet Ramesh

Ramesh (31), from an agricultural village in Nuapada, Odisha, is the backbone of his family. From sunrise to sunset, he stands in the paddy fields. At the end of the day, he often felt a dull, heavy ache in his groin, but he dismissed it as the simple price of hard work.

For five years, he and his wife, Sunita, prayed for a child. Sunita went through every test, and everything was perfectly normal. Finally, Ramesh agreed to be checked. He felt physically strong, but he didn’t realise that his “roots” were tangled. The dull ache he ignored was a physical condition called a Varicocele, which was acting like a broken radiator, slowly overheating his “seed bank” and stopping new life from taking form.

The Problem: The “Broken Radiator”

For a man’s body to produce healthy sperm, the testicles must remain about 2 to 3 degrees cooler than the rest of the body. To do this, nature provides a cooling system, a network of veins that pump warm blood away.

In a Varicocele, the tiny valves inside these veins break. Instead of flowing away, the warm blood pools and tangles, much like varicose veins in the legs.

This creates a localised “furnace.” The trapped heat not only stops sperm production, causing a low count, but it also damages the DNA of whatever sperm does survive.

The Causes

Gravity and Time: Long hours of standing in fields, factories, or at looms put intense pressure on these delicate venous valves.

Anatomical Angles: The veins on the left side of the male body meet at a sharper angle, making the left side much more prone to this pooling of blood.

Silent Progression: It often starts in a man’s late teens or twenties and slowly worsens over time, rarely causing sharp pain, just a “heaviness.”

What Tests Matter

A basic semen analysis will show low numbers, but it won’t tell you why. We have to look at the plumbing.

Scrotal Colour Doppler Ultrasound: A specialised scan to literally see the blood flowing backwards and pooling in the veins.

Sperm DNA Fragmentation (DFI): To measure exactly how much “heat damage” has been done to the sperm’s internal blueprint.

XAI Vascular Mapping: At Santaan, we use AI to analyse the exact severity of the blood reflux. This helps us predict with high accuracy whether repairing the veins will naturally restore fertility, saving the couple from needing IVF.

Treatment Options

Microscopic Varicocelectomy: A highly precise, minor day-care procedure. Using a microscope, we gently “tie off” the broken veins. The body instantly reroutes the blood through healthy veins, turning the “cooling system” back on.

Antioxidant Cooling: Using specific micronutrients (like CoQ10 and Zinc) for 90 days post-procedure to heal the heat-damaged tissue.

Supportive Lifestyle Adjustments: Wearing supportive undergarments during heavy physical labour to take the pressure off the healing veins.

FAQ

• If I have a varicocele, do I definitely need surgery?

Not always. If your sperm quality is still high and your wife is young, we might just monitor it. We only intervene if it is clearly a roadblock to your baby.

• How long after the repair will my count improve?

Sperm take about 72 to 90 days to grow. Most men see a dramatic improvement in their reports within 3 to 6 months.

• Is the pain normal?

A dull ache or a feeling like a “bag of worms” after a long day is the classic sign. Ignoring it won’t make it go away; fixing it protects your future.

Fatherhood is built on a strong, healthy foundation.

If your wait is long and you feel that silent heaviness, let’s check the roots.
